FAFSA (Free Application for Federal Student Aid):  The FAFSA or Renewal FAFSA should be completed by all students wishing to apply for federal, state, or need-based institutional aid. NC State’s school code for the FAFSA is 002972.

• NC State Institutional Financial Aid Application: The NC State Institutional Financial Aid Application form may be printed directly from the link above, requested from the NC State Office of Scholarships and Financial Aid, or one will be mailed to you automatically upon receipt of your electronic FAFSA data from the federal processing center.  This form must be completed the first time that a student applies for aid at NC State but does not need to be completed annually.  This form provides important demographic & enrollment information, a privacy release, and an opportunity for you to select a lender for Federal Stafford loans.
       
• Verification Worksheet:  If your application was selected for review in a process called "Verification," you must complete the Verification Worksheet and Worksheet ABC. In the verification process, we will be comparing information from your FAFSA with signed copies of your and your parent(s)' or spouse's Federal tax forms, W-2 forms, or other financial documents. Federal regulations require you to provide this information before you may be awarded Federal aid. If there are differences between your application information and your financial documents, we will correct your FAFSA information automatically and send those corrections electronically to the federal processor.

   
• Graduate Enrollment and Benefits Status Form:  The Graduate Enrollment and Benefits Status Form is required of all graduate students enrolled in master's or doctoral degree programs and must be completed each year that a graduate student applies for aid.  This form is used to gather information about your enrollment plans and determine what types and amounts of support you are receiving from your department.

   
• Federal PLUS Loan Information Form:  The Federal PLUS Loan Information Form is required of parents who wish to apply only for Federal PLUS loans and not for other types of assistance.  The form is not required if the student has completed a FAFSA.
   
• Federal PLUS Loan Request Form:  The Federal PLUS Loan Request Form is used by parents to request to borrow a Federal PLUS loan, select a PLUS lender, and to request the amount that a parent wishes to borrow through the PLUS loan program. 

• Policy on Satisfactory Academic Progress for Financial Aid Eligibility: The Policy on Satisfactory Academic Progress for Financial Aid Eligibility may be viewed and printed directly from the link above.  This policy describes the academic requirements that a student must meet to maintain continued eligibility for federal, state, and institutional funding. 
   
• Study Abroad Enrollment Agreement:  The Study Abroad Enrollment Agreement must be completed by any student who wishes to use financial aid while studying abroad. 
   
• Study Abroad Budget & Consortium Form:  The Study Abroad Budget & Consortium Form is required ONLY for students who are doing a study abroad program that is not sponsored by NC State University.
   
• Consortium Form:  The Consortium Form is used only by students who are enrolled in a degree program at NC State but are also taking approved coursework at another institution which will be applied toward a degree at NC State.
