Forms & Publications
If additional documentation is required to complete your aid application, we will contact you to request the appropriate information. Each of the forms on this page may be printed directly from the links below, or you may contact our office to request a paper copy.
Forms for all students:
- NC State Institutional Financial Aid Application must be completed the first time that a student applies for aid at NC State but does not need to be completed annually. This form provides important demographic & enrollment information, a privacy release, and an opportunity for you to select a lender for Federal Stafford loans.
- Scholarship Notification Form should be completed to notify our staff of any outside scholarships that you expect to receive.
- Update Form may be used to notify us of changes in your enrollment plans.
Forms for verification:
If your application was selected for review in a process called "Verification," you must submit the appropriate verification worksheets along with copies of your tax returns and W-2's. Federal regulations require you to provide this information before you can be awarded Federal aid. If there are differences between your application information and your financial documents, we will correct your FAFSA information automatically and send those corrections electronically to the federal processor.

Loan forms:
- Federal PLUS Loan Request Form is used by parents to request to borrow a Federal PLUS loan, select a PLUS lender, and to request the amount that a parent wishes to borrow through the PLUS loan program.
- Alternative Loan Letter is required of students who wish to borrow only private loans and do not plan to submit a FAFSA.
- Computer & PDA Purchase Request Form is used to request additional loan support to assist with the purchase of a computer.
Forms to report additional aid:
- Scholarship Notification Form should be completed to notify our staff of any outside scholarships that you expect to receive.
Donor Instructions Form may be given to the donor/sponsor of your outside scholarship so they know how to submit funds to NC State. - Graduate Enrollment Status & Benefits Form is required of all graduate students enrolled in master's or doctoral degree programs and must be completed each year that a graduate student applies for aid. This form is used to gather information about your enrollment plans and determine what types and amounts of support you are receiving from your department.
- Certification of VA Educational Benefits is required for students who will be receiving any veteran's educational benefits while enrolled.
- ROTC Scholarship Notification Form is required for students who will be participating in ROTC programs.
Special enrollment forms:
- Study Abroad Enrollment Agreement must be completed by any student who wishes to use financial aid while studying abroad.
- Study Abroad Budget and Consortium Form is required ONLY for students who are doing a study abroad program that is not sponsored by NC State University.
