Name and/or Address Changes
Address Change
You can update your address in the Human Resources/Payroll System through MyPack Portal. After signing in, click on "Employee Self Service", and then click on "Personal Information". Here you can change your home address, your email address and your home or cell phone numbers.
If you are unable to access MyPack Portal you can contact your department’s personnel representative for assistance.
Name Change
If you change your name, you will need to get a new Social Security Card from the Social Security Administration. Your Social Security number will stay the same and a new card will be issued showing your updated name. Complete the Social Security Card (SS-5) Application and submit proof of your identity as instructed. Visit the Social Security Administration website for more information.
After you receive your new Social Security Card, you need to present it to your department’s personnel representative for a name change in the HR System. It will need to be exactly as it is on the SSN card and you will need to complete a new I-9 employment eligibility form.
All foreign nationals (permanent and children) are required to report any change in address within 10 days to USCIS using Form AR11. Notify the International Employment office of your new address, too.
Benefit Vendor Address and/or Name Changes
By updating your address in the HR System and/or having your name changed in the HR System, the following vendors will be receive your address and/or name change automatically:
- Teachers’ and State Employees’ Retirement System
- State Health Plan
- MetLife Group Term Life Insurance
- Hyatt Legal Plan
- Liberty Mutual Long Term Disability
- Standard Long Term Disability
If you have the following benefits, you will need to go onto NCFlex's website at http://www.ncflexonline.org to make address and/or name changes. After you log into their system with your unique ID/password, simply click on the "My Account" icon in the upper right corner. You can then change your address and/or name. Make sure you click on the "Save" button.
