Benefits Annual Enrollment

You can make changes, enroll in or re-enroll in these benefit plans. Keep in mind if you are enrolling in some of the plans for the first time outside of your 30-day new hire window, you may be subject to waiting periods. Go to www.ncflexonline.org and log in with your ID and password that you created (not your unity ID). These changes can only be made online. If you do not have online access you may call the NC Flex customer support at 1-888-860-6118 to enroll in/change the benefit plans listed above or if you have online access issues. Changes made are effective 1/1/09. For more information about the plans and changes visit www.ncflex.org
If you are signing up for MetLife life insurance for the first time or adding a dependent(beyond 30 days of your hire date), you will need to complete the MetLife enrollment form and MetLife statement of health (evidence of insurability is required before approval is granted). If you wish to make changes to your existing MetLife coverage simply complete the MetLife enrollment form. If you currently have 1 times or 2 times salary coverage, you can move up one level with no statement of health required. Simply complete a MetLife enrollment form
To add a domestic partner to your existing MetLife plan, complete the MetLife enrollment form and a domestic partner declaration. If enrolling in MetLife for the first time with a domestic partner, you will need to complete the above forms along with a MetLife statement of health for each of you.
TheMetLife enrollment forms MetLife statement of health and domestic partner declaration can be:
- Faxed to the HR Benefits Office at (919) 513-2528 OR
- Sent via campus mail at HR Benefits, Campus Box 7215 OR
- Sent regular mail to HR Benefits, Campus Box 7215, Raleigh, NC 27695-7215 OR
- Dropped off at the HR Benefits Office located at 2711 Sullivan Drive (Administrative Services II Building, second floor)
If you wish to enroll in this plan for the first time or make changes, complete the Liberty Mutual enrollment form. No statement of health is required for those enrolling for the first time. However, if previously denied, a statement of health is required.
The Liberty Mutual enrollment forms and statement of health can be:
- Faxed to the HR Benefits Office at (919) 513-2528 OR
- Sent via campus mail at HR Benefits, Campus Box 7215 OR
- Sent regular mail to HR Benefits, Campus Box 7215, Raleigh, NC 27695-7215 OR
- Dropped off at the HR Benefits Office located at 2711 Sullivan Drive (Administrative Services II Building, second floor)
To enroll or make changes to your Hyatt Legal Plan, complete a Hyatt Legal Plan enrollment form.
The Hyatt Legal Plan enrollment form can be:
- Faxed to the HR Benefits Office at (919) 513-2528 OR
- Sent via campus mail at HR Benefits, Campus Box 7215 OR
- Sent regular mail to HR Benefits, Campus Box 7215, Raleigh, NC 27695-7215 OR
- Dropped off at the HR Benefits Office located at 2711 Sullivan Drive (Administrative Services II Building, second floor)
