may order DELTA videotapes, DVDs and CD-ROMs from NC State Bookstores using
a major credit card. Only students registered in the video sections
are allowed to borrow videotapes/CD-ROMs. Students wishing to pick
up videotapes, DVDs and CD-ROMs in person should come to the store without
placing the order online. Students that prefer to have the videotapes,
DVDs
or CD-ROMs mailed to them, but who are paying by check should call
the bookstore to make arrangements. The telephone number for the

Acceptance
of the terms of the DELTA Video Loan Agreement is required in order
to process the order.
DELTA
VIDEO/DVD/CD-ROM LOAN AGREEMENT
I
understand that enrollment in the DELTA Video/DVD/CD-ROM program requires
me to use course specific videocassette tapes, DVDs or CD-ROMs. I further
understand that any additional course materials or textbooks required
for this course are not a part of this agreement and must be purchased
separately. I agree to comply with the following conditions:
1.
The materials recorded on these Videotapes/DVDs/CD-ROMs is copyrighted
and may not be duplicated, transmitted or otherwise used except
for viewing by paid registrants of this course. Requests for exceptions
to these restrictions are to be addressed to DELTA at (919) 515-9030
2.
A security deposit of $50.00 per course will be payable in full
when the course Videotapes, DVDs or CD-ROMs are dispensed.
Videotapes or CD-ROMs can be shipped to students for an additional
non-refundable charge.
3.
A full refund of the security deposit will be returned to the student
if the Videotapes/DVDs/CD-ROMs are received by the deadline of
December 23, 2008;
provided the Videotapes/DVDs/CD-ROMs are received in an undamaged and
unaltered condition. If the Videotapes/DVDs/CD-ROMs are not
returned by the deadline, the deposit is forfeited and a hold is
placed on the student's University Records until NCSU Bookstore has
received all materials on loan. The replacement charge for damaged or missing
Videotapes/DVDs/CD-ROMs is $10.00 per videocassette/DVD/CD-ROM.
4.
The NCSU Bookstores must receive official written notification from
DELTA if an extension for delayed completion of this course is granted.
It is understood that simply receiving an incomplete in the course
does not entitle the student to a refund of his/her security deposit.
A written extension agreement with a deadline for completion signed
by the course instructor is required.
5.
If a student drops or withdraws from a course, all Videotapes/DVDs/CD-ROMs
on loan are to be returned immediately. A hold will be placed on
the student’s University Records until NCSU Bookstore has
received all materials on loan.
6.
Videotapes/DVDs/CD-ROMs may be returned in person or by certified mail
to: NCSU Bookstores, Campus Box 7224, Cates Ave, Raleigh, NC 27695-7224.
7. Textbooks can be returned to NCSU for full refund of purchase
price according to NCSU Bookstore’s Textbook return policy.
After the textbook return period ends, the bookstore will buy textbooks
from students through the textbook buy-back program. Detailed information
